When you submit an inquiry, we collect the contact details, service and location preferences, and message you provide. We use Google Firebase to store and process the request and Google Workspace to email you and the Oak Wellness team responsible for your inquiry. These details help us respond and coordinate next steps. This form is not for clinical intake.
